Job Summary

Make a meaningful impact in your community—join Acentra Health as a Clinical Evaluator (PASRR) in Seattle, Washington. In this hybrid role, you will conduct in-person preadmission evaluations and assessments throughout the Seattle area.

As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. . These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.

This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. All travel expenses—including mileage and parking are covered and reimbursed by Acentra Health. In addition, Acentra provides all necessary technology and equipment for your home office, including a laptop for use in the field. 

 

Job Responsibilities:

  • As a Clinical Evaluator (PASRR), you will conduct accurate screenings and evaluations, developing informed level-of-care recommendations based on your clinical assessments.
  • Complete precise PASRR reviews by assessing and determining the appropriate level of care based on patients’ physical, behavioral, and psychological functioning. 

  • Review medical records, and interact with healthcare professionals, patients, their families, and other support systems to conduct accurate screenings, evaluations and determinations Assess and analyze psychological, emotional, behavioral and physical functioning.
  • Ensure compliance with contractual and regulatory standards while developing level of care recommendations.
  • Assess and analyze patients' psychological, emotional, behavioral, and physical functioning to inform clinical decisions.
  • Provide well-informed level of care recommendations based on clinical evaluations.
  • Ensuring compliance with established workflows, including re-evaluations of records based on psychiatric input.
  • Collaborate effectively with healthcare professionals, patients, families, and internal stakeholders as needed.
  • Efficiently utilize computer equipment and web-based software applications to perform job duties
  • Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities. .
  • Efficiently prioritize and execute tasks with attention to detail.

 

Qualifications:

Required Qualifications, Knowledge, and Experience

  • Requires an active, unrestricted license in the State of Washington in one of the following fields: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Social Worker Independent Clinical License (SWICL), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Psychologist (LCP), or Licensed Mental Health Professional (LMHP).
  • Graduation from an accredited college with a relevant degree program with a minimum of 2–3 years of experience assessing individuals with mental health or psychiatric disorders, including complex differential diagnoses related to substance use, medical conditions, cognitive impairments, and dementia.

  • Working knowledge of Preadmission Screening and Resident Review (PASRR) requirements, including applicable Federal Regulations and state-specific rules.

  • Familiar with standards of practice for determining Level of Care, incorporating best practices in mental health, geriatrics, and conditions commonly necessitating long-term care placement.

  • Skilled in assessing and analyzing psychological, emotional, behavioral, and physical functioning to develop clinically appropriate Level of Care recommendations based on comprehensive evaluations.

  • Detail-oriented with strong organizational skills; able to prioritize and complete tasks efficiently within set timelines.

  • Proficient in computer-based software and tools, including Microsoft Office Suite (Word, Excel, Outlook, Teams).

  • Familiarity with public mental health and medical programs, with prior experience working with the geriatric population.

Compensation

The compensation for this role is $41.00 to $44.00 per hour

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
