Job Summary

Make a meaningful impact in your community—join Acentra Health as a Clinical Evaluator (PASRR) in Lincoln, Nebraska. In this hybrid role, you will conduct in-person assessments in nursing homes, hospitals, and community-based settings throughout Lincoln and its surrounding areas. 

As a Clinical Evaluator, you will conduct face-to-face PASRR Level II assessments to determine appropriate placements and next levels of care for individuals with complex needs. Your primary focus will be completing clinical evaluations for individuals with Serious Mental Illness (SMI), Intellectual Disabilities (ID), or Related Conditions (RC) across nursing homes, hospitals, and community-based settings. These evaluations are crucial in determining the next level of care, improving patient well-being, and supporting better health outcomes.

This PRN hybrid role offers flexibility and purpose, combining remote work from your home office with local travel as needed. All travel expenses—including mileage and parking are covered and reimbursed by Acentra Health. In addition, Acentra provides all necessary technology and equipment for your home office, including a laptop for use in the field. 

Job Responsibilities

  • As a Clinical Evaluator (PASRR), you will conduct accurate screenings and evaluations, developing informed level-of-care recommendations based on your clinical assessments.
  • Conduct assessments in various environments, including nursing homes, hospitals, and community settings, to understand and address the diverse needs of those we serve.
  • Adhere to the state's best standards of mental health practices, intellectual and developmental disabilities, geriatrics, and other conditions requiring nursing care services and supports.
  • Ensure compliance with contractual and regulatory standards while developing level of care recommendations
  • Utilize independent judgment, deep clinical expertise, exceptional communication skills, and problem-solving abilities to navigate complex situations, ensuring the best outcomes for members, providers, and clients.
  • Foster effective collaboration with a diverse range of healthcare professionals, patients, families, and support networks while monitoring personal metrics to ensure compliance with turnaround time requirements.
  • Leverage technology by utilizing the state's electronic system and our proprietary platforms to efficiently manage and track all assessment activities.
  • Stay informed on current literature, regulations, and research pertaining to long-term care, mental illness, medical conditions, and geriatrics.
  • Ensure compliance with necessary workflows to maintain quality and efficiency.
  • Efficiently prioritize and execute tasks with attention to detail.
Qualifications:

Required Qualifications, Skills, Knowledge and Experience

  • Active, unrestricted licensure in the state of Nebraska in one of the following fields: Licensed Clinical Social Worker (LCSW), Licensed Master Social Worker (LMSW), Licensed Marriage and Family Therapist (LMFT), Licensed Independent Clinical Social Worker (LICSW), Licensed Clinical Psychologist (LCP), Registered Nurse (RN), or another qualified Licensed Mental Health Professional (LMHP).
  • Graduation from an accredited college or university with a degree relevant to the field, and minimum of 2 + years of experience assessing and diagnosing individuals with mental or psychiatric disorders—including expertise in complex differential diagnoses involving substance use, medical conditions, cognitive impairments, and dementia.
  • Minimum 1 year of experience working with individuals with intellectual disabilities or related conditions.
  • Working knowledge of the Preadmission Screening and Resident Review (PASRR) requirements as per Federal Regulation and the state PASRR rules.
  • Skilled in assessing and analyzing psychological, emotional, behavioral, and physical functioning to develop appropriate level of care recommendations based on clinical evaluations.
  • Knowledge of psychiatric disorders as outlined by the latest Diagnostic and Statistical Manual in use in Kansas.
  • Strong communication skills with the ability to interact effectively with individuals and groups through verbal, written, and electronic formats.

  • Detail-oriented, with the ability to prioritize and execute tasks efficiently within established timelines.
  • Proficient in using computer-based software and Microsoft Office Suite (Word, Excel, Outlook, Teams, etc).

  • Familiarity with public mental health and medical programs is a plus.

  • Prior experience working with the geriatric population is preferred.

Compensation

The compensation for this role is $40.00 to $43.50 per hour

Based on our compensation program, an applicant’s position placement in the pay range will depend on various considerations, such as years of applicable experience and skill level.
